- Complete the application. You will choose the appropriate fall term to apply, and choose "Communicative Disorders and Sciences - PhD" from the programs drop down list.
- Unofficial transcripts of all undergraduate and graduate work.
- A minimum of three letters of recommendation by persons familiar with the student’s past academic, clinical, and/or research work, and who can attest to the student’s personal qualifications and potential for satisfactory completion of PhD-level work in the field. It would be helpful if the letters address the following topics:
- potential to complete the academic and research requirements for a doctoral degree
- ability to absorb complex knowledge and ideas, to integrate information across disciplines and to think creatively
- ability to communicate effectively in spoken and written form
- ability to work independently ability in research
- A letter of intent that is a statement of the student’s academic and professional goals, as well as your research interests.
- Current Resume or CV.

A $75. graduate application fee is required with your UB Slate application. This must be paid before formal review of your application.
